EPA RENOVATOR TRAINING COURSE-INITIAL TRAINING CONTRACT
All American Building Products, Inc. is pleased to provide you with the following
information concerning our EPA Renovator Training Course. Below are our Training
Contract and our student reservation form. Please call our office or visit our
website www.aabpinc.com for scheduled training
classes, satellite locations and rates.
WE MUST RECEIVE A COPY OF THIS AGREEMENT FROM EACH PARTICIPANT AT LEAST 5
BUSINESS DAYS BEFORE THE CLASS. PLEASE REGISTER ONLINE, OR FAX THIS FORM TO THE
FAX NUMBER ABOVE.

Trainings will be held in various locations depending on branch where
it is to be held.

Class duration is expected to be approximately 8 hours (all courses start
promptly at 8:00 a.m. and end at 5:00 p.m.) Time will be given for lunch
and periodic breaks. All American will provide snacks and lunch for attendees.

Students will be provided with a training manual which will be theirs
to keep.

All attendees will take a test in which they must
pass with a 70% score in order to successfully complete the class and receive
their certificate.

All American reserves the right to allow other attendees to participate in
the training course.

If training course is requested at a satellite location, requesting party
must provide adequate space for the class and hands on activities

All American must be notified 48 hours before the scheduled class date, if
any attendees will not be present. In failure to do so, participant will
be billed for the full price of the reservation. If notice is received within
48 hours, you may reschedule your class or request a refund.

Payments will be made in full at time of registration.

While every effort will be made to present certificates at the close of the
class, please allow up to 2 weeks for delivery of certificate.
I have read, accepted and understand the terms listed on this contract and those
provided to All American Building Products, Inc in the full training agreement.
**Please keep all fees confidential. All American Building Products, Inc gladly
accepts: Visa, MasterCard, Discover, American Express, Checks and Cash. If using
a credit card we will need the exact name on the card, billing address, w/ zip code,
CID code, expiration date, and associated phone number.**
